Family Ties AI is a preventative digital support platform that helps people stay independent at home while keeping authorised family members and carers connected and informed.

Support arrives through channels people already use — WhatsApp, SMS and phone calls. There is no new app to download and nothing new to learn.

Ahead of a hospital appointment, the platform can remind someone what they may need to take or prepare. Afterwards it can ask how it went, and record what matters in their personal journal for next time. That is the difference between a reminder and an alarm: it understands the context, and it comes back.

Day to day, that means regular wellbeing check-ins, reminders for whatever matters — medication and appointments, but equally posting a letter, checking the fridge, getting out for a walk — routine prompts, and ordinary conversation.

The journal remembers what you might not. What you did last week, who you spoke to, how you were feeling — written down as you go, and there when you want it.

Between the formal visits

Most of the risk in home care sits in the gaps — between a discharge and the first follow-up, between one carer visit and the next, between an appointment and whatever was supposed to happen after it.

Family Ties AI is infrastructure for those gaps. It supports coordination between formal interactions, reduces avoidable administrative burden, improves follow-up, and helps families and carers maintain continuity at home. Hospital-to-home and discharge support is where that matters most, and where a missed follow-up costs the most.

Keeping family in the loop

The family portal gives authorised family members a view of what matters — wellbeing over time, appointments and reminders, the calendar, and messages where they've been shared. If someone doesn't respond to an agreed check-in, a nominated family member is alerted so they can follow up.

What they see is decided by the person receiving support. Not by us, and not by the family. They choose what is shared and can change it at any time.

What it is not
